The clinical diagnosis of R-CPD is supported by the reduction of symptoms with BT injection, and BT into the CPM is an effective treatment, with benefits that frequently outlast the pharmacologic length of BT's activity. R-CPD can be diagnosed syndromically utilizing a symptom complex.

How do I know if I have R-CPD?

Nausea, especially after eating or drinking fizzy beverages in excess. Hiccups that hurt, again more frequently after eating. Shortness of breath. A person can be so buoyant that they lose their capacity to exercise, climb stairs, etc.

What causes a person to be unable to burp?

Failure to burp or belch results from the upper esophageal sphincter's (cricopharyngeus muscle) inability to relax and let go of the air "bubble." A muscular valve known as the sphincter surrounds the top portion of the esophagus immediately below the bottom portion of the throat passage. The cricopharyngeal sphincter typically contracts and only relaxes to let food flow through or during burping or belching. Gas gets retained in the esophagus in persons with the inability to burp disorder because the cricopharyngeal muscle cannot relax.

How can I treat when I am unable to burp or belch?

The cricopharyngeus muscle is injected with Botox to address this problem. This temporarily paralyzes this muscle to the point that burping can resume. The majority of patients will thereafter learn how to burp in around 3 months. The injection might need to be given again to certain people.
