Costovertebral  angle pain (CVA) is pain caused by touching the area inside the costovertebral angle. The CVA is formed by the 12th rib and the spine. Evaluation of stroke pain is part of the abdominal exam, and stroke pain often indicates kidney disease.

Susceptibility to stroke often indicates kidney disease, but it can be the result of other medical problems. Pain from stroke is common in acute pyelonephritis. Renal abscess, urinary tract infection, and ureteral reflux. Susceptibility to stroke is also present in patients with appendicitis and retroperitoneal abscess. In patients with sickle cell disease, a blockage of blood flow to the kidneys can cause a stroke.

The absence of stroke susceptibility in patients with low back pain supports a diagnosis other than renal disease.
