Office procedure note this 39-year-old patient returns to the office today because of a defect on the nail of his left big toe. he was seen three times in the last four months for this problem. this defect is very suspicious, and i felt it best to biopsy a portion of the nail plate and bed. the sample was sent to pathology. i told the patient that he will be called with the results, and then i will decide how to proceed. cpt code(s):
